Incoming Freshman
- Class of 2028 - Health Services Welcome Letter
- IL Certificate of Child Health Examination Form – for class of 2028: school physical completed on/after 8/21/2023
- IL Proof of Dental Examination Exam Form – by May 15th of their freshman year, students in grade 9 are required to have a dental exam on file. School dental examinations must have been completed within 18 months prior to the May 15 deadline. For the class of 2028, dental examinations completed on/after November 15, 2023.
- Dental Examination Waiver Form – if unable to have exam due to lack of/insufficient coverage
- NTHS Medication Form – required for all prescription and over-the-counter medications including ibuprofen, acetaminophen, and diphenhydramine
- Allergy Action Plan – for students with potential anaphylactic allergies
- Asthma Action Plan – for students with asthma
- Seizure Action Plan – for students with known seizure history or epilepsy
- Diabetes Medical Management Plan – for students with diagnosis of diabetes

Freshmen

Transfer Students
- IL Certificate of Child Health Examination form - for physical completed withing 1 year of transfer
- State of IL Eye Examination Report Form - for students transfering from outside Illinois, completed within 1 year prior to enrollment
- Allergy Emergency Action Plan - for students with potential anaphylactic allergies
- NTHS Medication Authorization Form - optional, for prescription and over-the-counter medications to be taken at NTHS
- Asthma Action Plan - optional, for students with asthma

All Seniors
- Documentation of MCV4/Menigitis immunization on/after student's 16th birthday - any official vaccination documentation is acceptable. Additional ISHA physical form is not needed.

All Athletes: Freshman, Sophomores, Juniors, and Seniors playing interscholastic sports
- IL Certificate of Child Health Examination form - due yearly. Physicals are valid for 395 days (one year, plus one month grace period). IHSA does not allow exceptions

Immunizations

In order to attend school, a record of current immunizations or a physician's statement of immunity must be on file. If an immunization is medically contraindicated, a physician must provide a statement specifying the reason. A religious exemption requires the filing of the Illinois Certificate of Religious Exemption form signed by parent and health provider. Please refer to Guidelines for Completion of the Certificate of Child Health Exam.
Meningitis (MCV4) vaccine: For Freshman: Requires one dose on or after the 11th birthday. For Seniors: Requires one dose on or after the 11th birthday and a second dose after the 16th birthday. If the first dose is given after the 16th birthday only one dose is required.
Religious Objections to Immunizations and/or Examinations: As of 10/16/15, parents or legal guardian must submit Illinois Certificate of Religious Exemption form signed by parent and health care provider. Form will be required to start classes.
Past New Trier Attendees/Graduates Requesting Immunization Records
If you would like a copy of your immunization records, please email healthservices@nths.net or fax (847.835.9852) a request to Health Services with the following information:
- Your name/maiden name if applicable
- Date of birth
- Year of graduation
- Your signature
- Where records need to be sent
Immunization records can also be accessed through the Records Office.
COVID-19 & Respiratory Viruses

The Centers for Disease Control and Prevention have updated guidance regarding COVID-19 and other respiratory illnesses. New guidelines can be found here.
New Trier is adopting these new guidelines, which state that you should stay home if you have respiratory virus symptoms that aren’t better explained by another cause. These symptoms can include: fever, chills, fatigue, cough, runny nose, and headache, among others.
When sick with a respiratory virus, the updated guidance recommends staying home and away from others. For people with COVID-19, treatment is available and can lessen symptoms and lower the risk of severe illness. Contact your medical professional for treatment options. The recommendations suggest returning to normal activities when, for at least 24 hours, symptoms are improving overall, and there is no fever, without use of a fever-reducing medication such as ibuprofen, acetaminophen or aspirin.
Once resuming normal activities, you are encouraged to take additional prevention strategies for the next 5 days to curb disease spread, such as taking more steps for cleaner air, enhancing hand hygiene, wearing a well-fitting mask and maintaining distance from others.
