• IL Certificate of Child Health Examination Form – Class of 2029: school physical completed on/after 8/20/2024 
• IL Proof of Dental Examination Exam Form – Class of 2029: dental examination completed on/after November 15, 2024. 
• Dental Examination Waiver Form – if unable to have exam due to lack of/insufficient coverage
• NTHS Medication Form – required for all prescription and over-the-counter medications including ibuprofen, acetaminophen, and diphenhydramine
• Allergy Action Plan – for students with potential anaphylactic allergies
• Asthma Action Plan – for students with asthma
• Seizure Action Plan – for students with known seizure history or epilepsy
